Effect of primary care based chronic disease management program provision characteristics on medication adherence in hypertensive and diabetic patients: Focusing on the implication for operation

Abstract
Objectives: The purpose of this study was to investigate the effect of provision aspects of local clinic based chronic disease management program on medication adherence of hypertensive and diabetes melitus patients.

Methods: A 1-year follow-up study was conducted using data from National Health Insurance Service was used for analysis. Medication adherence, calculated by medication possession rate, was used as a dependent variable. Chi-squared test and multiple logistic regression were performed to analyze the differences and influencing factors of medication adherence among hypertensive and diabetes mellitus patients participating in chronic disease management program.

Results: Hypertensive patients who received education 6 time or more(OR: 1,541, 95% CI: 1.188-1.999) and managed 3 times or more(OR: 1.500, 95% CI: 1.042-2.159) in one year were more likely to be medication adherent. Diabedic patients who received education during more than 20 minutes per one time(OR: 2.452, 95% CI: 1.249-4.811) were more likely to be medication adherent.

Conclusions: Based on the result of this study, payment system of chronic disease management program shoud be redesign for improving effectiveness.
